Skilled Nursing

Skilled nursing services are performed by either an RN or LPN and include an assessment of the patient's physical, mental and psychosocial status which determines the patient's proposed treatment plan. Treatment plans are then coordinated and approved by the patient's physician and may include, but are not limited to the following services: pain management, wound care, medication management and instruction, diabetic monitoring and instruction, disease specific treatment and education.

Physical Therapy

A Physical Therapist will evaluate a patient's range of motion, strength, ability to transfer, and risk for falls. Physical Therapy may be recommended to assess a patient’s functional ability, help provide pain relief, prevent disability, develop or improve a patient’s gait, balance, endurance and strength, maximize a patient’s functional capabilities, recommend assistive devices and home modifications to promote safety and independence, or provide training with a new prosthesis.

Occupational Therapy

An Occupational Therapist may be indicated in order to maximize a patient’s ability to perform activities of daily living using exercises to increase strength, dexterity and cognitive ability. Occupational Therapy focuses on teaching the patient to compensate for permanent or temporary loss of function to promote continued independence in activities such as bathing, dressing, cooking, and eating, performing housework, and managing personal business. Our Occupational Therapists are experienced in treating many types of conditions, including hand / shoulder injuries, cognitive difficulties, paralysis due to stroke or injury, limitations in movement, and visual challenges such as macular degeneration, low vision, etc.

Speech Therapy

Speech Therapy focuses on receptive language (the ability to understand words spoken to you). A Speech Therapist may evaluate for swallowing ability, appropriate type of diet, comprehension and articulation of verbal, written, and graphic expressions, hearing loss, cognitive retraining, memory retraining, and/or language disorders.

Medical Social Worker

Medical social workers assess the psychosocial functioning of patients and families and intervene as necessary. Interventions may include connecting patients and families to necessary resources and supports in the community, providing psychotherapy, supportive counseling, or grief counseling, helping a patient to expand and strengthen their network of social supports, or to assist the family to find alternative placement options in order to ensure the patient's safety and well-being.

Home Health Aide

Home Health Aide services are provided, under the supervision of a licensed professional, to help maximize a patient’s functional ability by providing hands-on personal care. Their level of assistance is planned by you, your family, and your nurse or therapist. Services that can be provided include bathing and dressing assistance, preparation of light meals, minor housekeeping tasks, and assisting with personal hygiene.
